About the Job
Responsibilities:
Working under moderate supervision, delivers care to hospice patients in neonatal, pediatric, adolescent, adult, and geriatric age group in their places of residence. Job duties include but are not limited to:
- Uses the nursing process to assess, plan, implement, and evaluate care while maintaining standards of professional nursing
- Provides individual care plans for each patient, advocate for patient welfare, serve as liaison between patient, family and health care providers
- Documents case management on an ongoing basis, identify and addressing client risk factors and or obstacles to care
- Identify client needs, connect client to services and resources to meet established goals
- OASIS and HIS and comprehensive assessment documentation of each patient
- Perform all applicable skilled care
- Monthly audits
- Collaboration between all disciplines to organize and plan care at home
- Education to client, caregiver and or family.
- Bereavement assessment
- Interdisciplinary communication with IDG team
- Participates in weekly IDG meetings with team
- Spiritual assessment
- Pain and Symptom Management

Qualifications:Graduate of a registered nursing program and possessing Indiana RN licensure; previous home care or hospice experience preferred. Required:
- Basic understanding of the related behaviors and needs appropriate to age and diagnosis of patients served in order to develop and provide and individualized plan of care
- Good interpersonal and communication skills
- Excellent decision-making skills and initiative to make independent judgments with the framework of established policies and procedures
- Knowledge of Medicare, State and other rules and standards related to Hospice
- Ability to drive and possess valid driver's license
- Ability to push/pull/lift/carry heavy objects
- Ability to operate computer and other technical medical equipment
- Maintains AHA BLS
