Enhanced Home LNA- Part-Time
$3000 Sign-on Bonus
RiverWoods Exeter is seeking a Part-Time 20 hour a week LNA to work varying hours M-F to support our Wellness clinic and the needs of our Independent Living Residents! The primary role will be to provide administrative support to RWE Wellness Clinics and Home Health Program including scheduling resident visits, calendar management, billing, developing educational materials and other administrative tasks as delegated by the Nurse Practitioner or Wellness Nurse, as well as provide homemaker and personal care services for independent residents of RiverWoods Exeter who are receiving Home Health Services. Hours can vary however typically would be weekdays between 9a and 2:30p. We are looking for team members who will bring their best selves and specialty skills to our organization. Responsibilities
- Organizes daily schedule of resident home health visits, updating as needed based on changing needs and priorities.
- Organizes upcoming weekly schedule of resident visits, making reminder calls to residents as needed.
- Keeps record of services provided by home health program and submits billing information on a weekly basis.
- Participates in relevant resident educational sessions; provides verbal and written education about home health program offerings.
- Provides administrative support to the Wellness Clinics as needed, which may include filing, organizing, developing new education materials, making phone calls and other tasks as delegated by Nurse Practitioner/Wellness RN.
- Performs intake sessions with new residents of home health program, including reviewing and signing appropriate consent forms. Collaborates with Nurse Practitioner/Wellness RN to complete appropriate assessments and service plans.
- Completes assigned daily resident home health care services tasks as agreed upon by the resident and a designated representative of the home health program.
- Documents daily services provided to residents of home health program in the electronic medical record.
- Reports and documents observable changes in the residents’ function, cognition, daily abilities and behaviors.
- Reports immediately and documents any unusual incidents to the Nurse Practitioner/Wellness RN.
- Recognizes and respects residents’ need for privacy and comfort and respects the dignity of residents while performing necessary care.
- Maintains resident, employee, and community confidentiality at all times.
- Adheres to the Residents Home Care Bill of Rights.
- Contacts residents of home health program to schedule home visits based on resident needs and availability.
- As the primary calendar manager, communicates and collaborates with other home health staff as needed to ensure timely resident visits.
- Assists residents with activities of daily living such as grooming, dressing, bathing, in accordance with resident care agreement.
- Checks vital signs as directed by Nurse Practitioner/Wellness RN.
- Assists residents with homemaker services such as, but not limited to: laundry, dishes, changing bed linens, light housekeeping, trash and recycle removal, meal prep, organization of kitchen, closets, bathroom, etc.
- Provides companionship and emotional support to residents receiving home health services.
- Collaborates with rehab department to assist and support residents in continuing home exercise programs.
- Maintains cleanliness and orderliness of resident apartments.
- Provides escorts to medical appointments as available.
- Recognizes and takes steps to remedy safety hazards.
- Communicates effectively both verbally and in writing.
- Adheres to RiverWoods community safety, organizational and ethical standards.
- Adheres to RiverWoods community policies, protocols, and procedures including infection control standards.
- Functions effectively as a member of a team.
- Maintains professional decorum at all times.
- Demonstrates a commitment to customer service.

Qualifications
Active NH LNA License
BLS Certification Required
Prior experience working directly with senior citizens is strongly preferred
