River Valley Family Health Centers is looking for a Health Education Specialist to join our clinics in Montrose or Delta Counties.

A day in the life of an Health Education Specialist may include:
- Assessing the health needs of patients at River Valley, in collaboration with the integrated health care team,
- Developing programs, materials, and events to teach people about health topics, such as managing existing conditions,
- Collecting and analyzing data to learn about needs and to improve our programs and services,
- Providing health related education to patients and their families, and teaching patients chronic care management through Annual Wellness visits,
- And so much more!
Compensation and Benefits
- $22.00-26.78/hour, depending on experience and certifications

Qualifications
- High School Diploma or equivalent.
- Completion of an approved educational program for Licensed Practical Nurses (LPN) with current licensure in the state of Colorado and proof of Continuing Education is preferred. Will consider a candidate that has at least two years of health education experience.
- Knowledge of safety and clinical standards.
- Ability to maintain positive working relationships with team members, patients, families, and others.
- Ability to utilize a computer for time keeping, documenting electronic medical records, and to complete follow-up tasks.
- At least one year of experience in an ambulatory health care sitting is preferred.
- Ability to maintain confidentiality.
- Ability to travel between clinics as needed.
- Bilingual candidates are preferred.
This position requires long periods of standing and walking, sitting at a computer, the ability to lift, carry, push, and pull up to 35 pounds, etc. Employees are exposed to blood, body fluids, and other occupational hazards, and are required to complete mandatory vaccines, such as Tdap, Hep A, Hep B, MMR, Varicella, and the yearly flu vaccine.
