East Bay Asian Local Development Corporation (EBALDC) is seeking a HIPP Case Manager in Oakland, CA.

Title: HHIP Case Manager - (Housing and Homeless Incentive Program)
Department: Resident & Community Services
Reports to:  Associate Director of Resident Services
Location:  Oakland, CA
Schedule: 37.5 hours/week; some evenings & weekends
Salary: $75,000 - $90,000 annually ($38.46 to $46.15/hour), depending on experience


Position Summary
The HHIP Case Manager supports individuals facing housing stability challenges, providing short-term case management, therapy, and crisis intervention to ensure residents maintain stable housing and improve quality of life. Stationed at San Pablo Hotel, a 144-unit property for seniors aged 55, the role involves engaging formerly homeless and/or MediCal eligible residents with high needs, including substance use, mental health issues, and complex medical conditions. This two-year, grant-funded position collaborates with Resident Services and Property Management teams to enhance resident safety and build overall community cohesion to support housing stability and self-sufficiency.
Key Duties and Responsibilities
Case Management (50%)

• Collaborate with property and resident services teams to identify and engage residents in need of support.
• Develop and implement Individualized Action Plans addressing housing stability, medical care, and self-sufficiency.
• Connect residents with community resources, entitlement programs, and treatment providers.
• Monitor progress through regular check-ins and adjust plans to ensure goal achievement.

Therapy and Counseling (10%)

• Provide individual counseling using evidence-based approaches like motivational interviewing and harm reduction.
• Facilitate psycho-educational and support groups to improve resident mental health and coping strategies.

Crisis Management (10%)

• Conduct mental health assessments, address risks, and respond promptly to crises.
• Collaborate with internal and external providers for comprehensive care and safety.
• Remain on-call after hours for emergency response.

Documentation and Reporting (10%)

• Maintain accurate case files and ensure compliance with organizational and HIPAA standards.
• Prepare required reports and participate in quality assurance efforts.

Other Duties (20%)

• Mentor resident services staff and support difficult resident situations.
• Attend organizational and community meetings, as well as resident events.
• Contribute to program development, evaluation, and implementation of policies.

Requirements

• Licensure: California LCSW, LMFT, or LPCC; ASW considered but clinical supervision unavailable. Qualified to provide clinical supervision to MSW’s, ASWs, and MFTs who are working towards licensure with preferred.
• Experience: 3 years in mental health, substance use, or social services; 5 years' experience in a housing setting preferred.
• Skills: Proficient in Microsoft Office, Salesforce strong communication and crisis management, and commitment to harm reduction. Experience with HIMS preferred.
• Other: Flexibility for occasional night/weekend work, valid CA driver’s license, and background clearance.

Workplace Environment

This role involves working with individuals who may have complex medical, psychological, and social needs in an SRO setting. Exposure to emotionally intense situations may occur. The workplace is a congregate living setting, where there may be potential exposure to common environmental pests associated with shared living spaces.
