We are thrilled to offer an opportunity for a Nurse Practitioner (NP) or Physician Assistant (PA) to join our compassionate primary care team, delivering exceptional care to patients across our outpatient offices in Connecticut. Key Responsibilities:
- Comprehensive Patient Care: Conduct thorough exams, take medical histories, and manage both acute and chronic conditions.
- Diagnostic Testing: Order and interpret diagnostic tests to inform patient care and treatment plans.
- Collaborative Care Plans: Develop individualized treatment plans in partnership with patients and their families.
- Specialist Referrals: Facilitate referrals to specialists as needed for continued care.
- Care Coordination: Work with the healthcare team to enhance the overall delivery of care and improve patient outcomes.
- Quality Improvement: Actively participate in initiatives like Patient-Centered Medical Home (PCMH) and Accountable Care Organization (ACO) programs to improve care processes and quality measures.
- Compliance & Documentation: Ensure adherence to billing, coding standards, and proper documentation practices.
- On-Call Coverage: Provide on-call support for the primary care group, approximately once every eight weeks.
Qualifications:
- Education: Graduation from an accredited Nurse Practitioner (APRN) or Physician Assistant (PA) program.
- Licensing: Current, unrestricted license to practice in Connecticut.
- Certifications: Active DEA registration and Controlled Substance Certificate.
- Experience: Minimum of 1 year of experience in a primary care setting is required.
