Position Description: The role of the Service Priority Specialist II (SPS- II) is to assist families experiencing a housing crisis by problem solving, identifying resources, offering support and/or assessment through the community coordinated entry system at the Family Housing Hub (FHH). The SPS- II position is responsible for providing client navigation services and screenings for families experiencing homelessness with the purpose of diverting families to safe housing options and/or to connect them to the community emergency shelter waitlist.
Essential Duties and Responsibilities:
Direct Client Care
- Provides resources, support, and information to households and assists households in creating short-term plans to solve immediate housing crises.
- Engages in problem-solving techniques and housing stability planning with households including preventing families from entering the homeless services system when appropriate.
- Completes assessments with households in a professional, trauma-informed, and accurate manner.
- Collects all required information from households to insure appropriate connections to resources and housing.
- Serves as liaison between household and partner agencies to assure continuity of care.
- In collaboration with partner agencies, assists clients in accessing all necessary documentation for program eligibility.
- Assists with triage at FHH front-desk and phones, greets clients, and provides high level screening upon initial contact.
Family Housing Hub Operations
- Staffs satellite locations and off-site screening events.
- With the FHH team, maintains up-to-date listings of community, employment, and housing resources.
- Clearly and consistently communicates program details and client navigation information to external partners.
- Assists in office administration including client record upkeep, data entry, and information inquires.
- Supports program Coordinator with customer service responsibilities by fielding client concerns and external partner inquiries.
Documentation, Data Quality, and Outcomes
- Assures data is complete, accurate, and on-time in accordance with UMOM's data quality standards.
- Focuses on achieving agency outcomes of diversion from homeless system, data quality, and program efficiency.
Qualifications and Competency Requirements:
Experience and Education
- Bachelor's degree required, ideally in a related field
- Minimum 3 years of experience working with at-risk populations and/or case management required
- Demonstrated knowledge of Motivational Interviewing and Trauma Informed Care, preferred
- Knowledge of non-profit sector; experience working with underserved or disadvantaged populations is preferred
- Knowledge of community resources and understanding of community health and social service system is a plus
Computer Skills
- Basic computer literacy required, including data entry, web navigation, typing and text editing, and the ability to learn operations in new software systems
- Proficiency with Windows operating system, Microsoft Word, Excel, and Outlook
Compliance
- Valid Level One Fingerprint Clearance Card or the ability to obtain one
- Valid AZ driver's license and a driving record that falls within UMOM's policy
- 50/100 level of car insurance coverage (mileage reimbursement available)

Work Schedule: The typical schedule for this position is Monday-Friday. This position is currently working a hybrid schedule - 4 days in office and 1 day remote.
