Discharge Planner - Cornerstone of Rhinebeck

Cornerstone of Rhinebeck is a short-term, inpatient chemical dependency treatment program for adults. We are currently seeking Per Diem Discharge Planner to work directly with patients.

Requirements:

• High School Diploma with two or more years of education and/or experience in chemical dependency counseling
• One to two years’ experience in discharge planning preferred. 
•  Ability to perform case management tasks with minimal supervision. 
• Must be computer literate.
• Prior experience with Electronic Medical Records.
