Discharge Planner

Our client is in beautiful Fort Myers, FL and looking for qualified professionals to join their team! They are a well establish non-profit that brings much needed services to the community.

The Position: Our client is looking for a Discharge Planner to join their established team in the Fort Myers area. They are offering the following compensation and benefits:

• $17.00/hr-$22.00/hr Based On Experience
• $1,000 Sign On Bonus
• Full Benefits - 60 Days After Start Date (Health, Vision, Dental)

Requirements:

• Minimum Bachelor’s Degree in Social Work, Psychology, Criminal Justice, Nursing, Rehabilitation, Special Education, Health Education, or a related field
• 1 Year Experience Working in a Behavioral Health Setting
• Proficient in Windows Operating Systems- Required
• Ability to Learn Electronic Health Records (EHR)
• Excellent Computer Skills
• (4) 10 Hour Shifts (7a-5p or 10a-8p)

Responsibilities:

• Effectively provides clinical and psychosocial services to clients
• Dependably coordinates and facilitates the development of individualized care plans and actively participates in interdisciplinary team meetings
• Provides thorough and accurate discharge planning information for initial care plans
• Ensure patients receive up to date resources in the community as a part of their discharge planning process
• Provided thorough and accurate intake screening on new admissions including obtaining appropriate collateral data
• Regularly and safely assists with de-escalation of agitated patients using approved TEAM
• Reliably completes other related duties as assigned
