Director of Social Services-Nursing Home

Located in Staten Island, NY 

Salary: $90K - $100K annually; Based on Experience

Responsibilities:

• Plans and directs a comprehensive social services program which provides support services for residents and families.
• Maintains adherence to all relevant state and federal regulations related to social services delivery.
• Responsible for coordinating all admissions and discharges to ensure facility census is kept at its maximum capacity.
• Tracks key performance indicators, generating reports on program effectiveness, and using data to inform decision-making.
• Participates in an interdisciplinary team approach to better serve the needs of the facility population.

Qualifications:

• Must have a MSW
• Must have at least 2 years of current Director of Social Work experience
• Must have current nursing home/LTC experience
• NYS PRI and Screen certification preferred
• Must have some knowledge and understanding of PDPM
• Strong written and verbal skills
