Director of Social Services - Healthcare Nursing Center

The Director of Social Services will provide social services to our residents which may include the admission process, facility adjustment, psychosocial needs, resident grievances, family grievances, MDS assessments, counseling, care plans, discharge planning, facilitate discharge procedures, Mass Health applications, policies and procedures, federal and state compliance. JOB DESCRIPTION

• The Director of Social Services “DSS” manages the psychosocial, mental, and emotional needs of residents residing in a skilled nursing center
• The DSS will develop and/or aide in the access of services
• Services will be provided in accordance with the National Association of Social Workers (NASW) Code of Ethics along with strict adherence to government regulations

QUALIFICATIONS

• The nursing home social worker must have skills in communication, assessment and social work methods and techniques
• Work effectively with a variety of disciplines in an individual and team setting
• Possess a working understanding of social systems along with the ability to implement pertinent state and federal regulations
• Licensed Required
