PeaceHealth is seeking a Diabetes Educator - Pharmacy Transitional Care for a Part Time, 0.60 FTE, Day position. The salary range for this job opening at PeaceHealth is $38.38 – $57.58. The hiring rate is dependent upon several factors, including but not limited to education, training, work experience, terms of any applicable collective bargaining agreement, seniority, etc.
Job Summary
This position is responsible for providing individualized or group, goal directed diabetes management and education for patients and their families according to the Scope of Practice, Standards of Practice, and Standards of Professional performance for Diabetes Educators.
Details of the position
+ Collaboratively develops educational goals, learning objectives and a plan for education content and teaching methods for program participants.
+ Provides diabetes self management training to patients in the clinic or hospital setting. Develops an individualized follow-up plan with each patient.
+ Evaluates effectiveness of educational services provided by measuring attainment of learning objectives. Conducts a follow-up assessment upon completion of the program.
+ Documents patient care in the medical record according to standards.
+ Participate in the development of training materials. Contributes to and participates in continuous quality improvement processes.
+ Evaluates ability to obtain diabetes related medications and testing equipment and related supplies.
+ Maintain and update education materials and keeping current with operational guidelines of monitoring devices and glycemic medication delivery systems.
+ Performs other duties as assigned.
What you bring
+ Bachelor's Degree Required: Bachelor of Science in Nursing or
+ Bachelor's Degree Required in a related field of study (for Dietitians). Caregivers who earned their RD license before 1/1/2024 when a bachelor's degree was sufficient will be grandfathered.
+ Minimum of 1 year Required: Experience in general medical nursing or dietetics with a variety of chronic conditions, including diabetes, cardiovascular, and renal disease
Credentials
+ Required: Registered Nurse - Washington
+ Required: Registered Dietitian National RD and state credential, as listed below:
+ May be required: Certified Dietitian - Washington
+ Required: Certified Diabetes Care and Education Specialist (CDCES)
