What you’ll be doing: As our site based Day Habilitation Manager, you will be assisting in the day to day organizing, monitoring & supervision of Day Services, Day Hab Without Walls area and/or the Pre-Vocational Program as needed.
You will be responsible for…
+ Mentoring and monitoring Day Habilitation staff (Direct Support Professional)
+ Assists and directs staff in development and delivery of all service activities
+ Intervenes and assists with individuals while maintaining a safe environment
+ Assists in development and implementation of behavior modification programs as needed

+ Bachelor’s Degree plus (1) yr full time experience in human services OR
+ Associate’s Degree plus (2) yrs full time experience in human services OR
+ High School diploma or equivalent plus (5) yrs full time experience in human services
+ Valid NYS Driver’s License & approved driving record

Position Summary:
The Day Habilitation Manager, under the supervision of the Coordinator of Day Services, assists in day to day organizing, monitoring, and supervision of Day Habilitation, Day Habilitation Without Wall area as needed. The Day Habilitation Manager performs other duties, as assigned, to include the supervision of staff.
Reporting Relationship:
Receives direct service supervision from the Coordinator of Day Services. Develops and maintains close working relationships with the other day habilitation support staff.
Job Duties and Responsibilities:
+ Individual Skills Training:
+ Mentors and monitors Day Habilitation staff as they:
+ Assist program participants with A.D.L. skills development and personal care issues, as needed.
+ Monitors break and lunch periods. Provide assistance with eating, use of adaptive equipment for eating, supervision and training in this area.
+ Spends a majority of program hours working in direct interaction with, or observation of, program participants.
+ Plan and implement activities in the community. Using, when possible, the natural supports/activities that are available in the community.
+ Monitor expected outcomes on a daily basis. Record data and summarize progress on a monthly basis.
+ Participate in intake meetings, semi and annual review meetings and assist in completing reviews for all treatment plans.
+ Contact families and residential staff regarding program needs.
+ Perform other duties as assigned.
+ Service Management:
+ Assists and directs staff in development and delivery of all service activities (including documentation) of Day Habilitation. Assures the quality and continuity of services to individuals is consistent with agency policies and regulatory requirements.
+ Provides input to the Program Manager in developing individual Habilitation Plans to ensure such plans are both consistent with the individual’s needs, preferences and personal goals to enable them to live, work, socialize and participate in the community and such plans are consistent with the philosophy and resources of Agency’s Programs.
+ Make recommendations to the Coordinator of Day Services regarding placement and termination of all individuals to special programs unit.
+ Under Supervision, ensures maximization of units of service through monitoring of attendance within special programs unit.
+ Determines need for equipment and supplies and makes recommendations to Coordinator of Day Services.
+ Ensures units of service through monitoring of individual’s attendance.
+ Determines need for equipment and supplies and makes recommendations to Coordinator of Day Services.
+ Assures safety of individuals through regular monitoring of program environment.
+ Monitors and maintains individual's financial accounts and balances as needed.
+ Coordinates and distributes petty cash fund appropriately.
+ Coordinates transportation of individuals to recreational activities as scheduled. This includes the signing out of vans and cooperating with other departments.
+ Intervenes with individuals' inappropriate and/or potentially injurious behavior in the least restrictive manner possible while maintaining a safe environment. Assists in development and implementation of behavior modification programs as needed.
+ Assists Coordinator of Day Services to ensure that all Day Habilitation staff receive all mandatory training. Assists in annual re-training of staff as needed.
+ Completes routine paperwork responsibilities and other duties as assigned by the Coordinator of Day Services. Receives supervision, as needed, from the Coordinator of Day Services
+ Service Supervision:
+ Coordinates, interviews and assists Coordinator of Day Services in hiring, orientation and termination of direct care staff.
+ Assures supervision of individual staff persons on a daily basis consistent with agency policies. May assign staff to other groups or areas as needed.
+ Evaluates staff performance annually. Document by using agency performance evaluation guidelines and procedures and makes recommendations to Coordinator of Day Services
+ Coordinates the schedule of all program activities for program participants to assure that services are provided and consistent with individual’s interests and plans.
+ Coordinates the use of staff benefit time, consistent with agency policy. This includes vacation days, personal leave days, etc.
+ Performs Other Duties As Assigned:
+ Monitors the maintenance of service, statistical and administrative records.
+ In conjunction with Coordinator of Day Services, participates in staff meetings for the purpose of discussing regular individual progress, workload review and dissemination of information.
Qualifications and Education Requirements:
Bachelor’s Degree PLUS one year full-time experience in human services.
OR
Associate’s Degree PLUS two years full-time experience in human services.
OR
High school or equivalent diploma PLUS four years of experience in human services.
PLUS
Supervisory experience is preferred.
Have the ability to act as an advocate for program participants.
Possess a Multi-Media First-Aid Certification and Strategies for Crisis Intervention and Prevention-Revised Training.
A valid New York State Driver’s License and approved driving record.

+ Equipment Operation:
The ability to operate office equipment, such as, computer, copier, fax machine, etc.; operate program equipment, such as, lifts, standards, exercise equipment, sensory equipment, etc.; operate agency buses and vans to include use of wheelchair tie down systems; etc.; Use household cleaners, chemicals, and ability to come in contact with normal household dust/dirt.
