Responsibilities:
Under supervision, the Continuing Care Planner serves as a liaison in relation to all case management and discharge planning within the facility.
Essential Duties:
- Work cohesively with the Clinical Outreach Team in the coordination of transitions in levels of care or release planning.
- Meet with families, as needed, during the treatment process to explain continuing care procedures and processes.
- Coordinate with clinical team, utilization review specialist, families, outside consultants, and continuing care facilities regarding patients’ discharge dates, level of care recommendations and transition plans.
- Initiate involvement with patient, families, and staff members as appropriate to formulate a discharge plan and continuing care treatment.
- Assist patients and families with the identification of a support network, instructions for future care, follow-up and concerns, and requests regarding care upon discharge.
- Assist patients and families with completion of all clinical documentation needed for transition to their next programs or social service organizations.
- Meet with patients and families to discuss placement options, organize records for transition, and provide advice regarding community assistance organizations, transportation options, etc.
- Coordinate and participate in educational and training activities related to continuing care planning for patients, including travel to visit programs and facilities both locally and nationally.
- Minimum of one (1) experience in a clinical setting.
- High school diploma or GED required.
- Knowledge and experience in the development of mental health discharge plans and coordination with community service providers, preferred.
- Bachelor’s degree in psychology, counseling, or sociology preferred.
Salary Range for this position is $17.00 - $22.12 an hr based on education and experience.
