Community Engagement Specialist
Full Time Days/8am-5pm/$21-$22 per hour based on experience
Lutheran Social Services of Central Ohio is currently seeking a Community Engagement Specialist for LSS Health Center (formerly Health Center at Faith Mission) in Columbus, Ohio. What will I do as the Community Engagement Specialist with Lutheran Social Services?
- Provides education regarding chronic illness and disease states to patients and their families to increase compliance with treatment protocols.
- Attend workshops, conferences, and other professional development opportunities relevant to HIV outreach and public health.
- Host workshops, health fairs, and community events to educate individuals and groups about HIV, STI prevention, and available health services.
- Work with healthcare providers, social service organizations, and other stakeholders to coordinate services and referrals.
- Promote anti-stigma campaigns and advocate for community acceptance of individuals living with HIV.
- Coordinates patient care with other members of the healthcare team, including primary care, mental health, dental, vision, behavioral health, and health education.
- Conducts assessments to determine patients' needs and preferences (e.g., PRAPARE).
- Conducts HIV Point of Care testing on site and at community events
- Enrolls patients into Patient Assistance Payment Interventions (PAPI) for payment assistance.
- Submits claims to Patient Assistance Payment Interventions (PAPI) at Ohio Department of Health and serves as liaison between health center and billing department.
- Engage with individuals and groups to encourage participation in HIV testing, treatment, and prevention programs.
- Provide pre- and post-test counseling for individuals undergoing HIV testing.
- Discuss test results confidentially and provide appropriate referrals for medical care or support services.
- Maintain accurate records of outreach activities, client interactions, and referrals in compliance with confidentiality standards.
- Contributes to the completion of regular reporting through quantitative reports, narrative summaries, and other relevant outputs.
- Works with volunteers in the health center to provide education and support.
- Attend relevant training courses/webinars and meetings as assigned.
- Provide feedback for the improvement of the Health Education Program.
- Participate in community-based outreach events as needed.
Requirements for the Community Engagement Specialist with Lutheran Social Services:
- Bachelor or Associates Degree in healthcare, social work, public health, health education or related field preferred.
- Community Outreach experience.
- HIV/PrEP education advocacy experience.
- Electronic Health Record experience preferred.
- Experience with data and reporting.
- Proven ability to build relationships with diverse populations, including those at high risk for HIV.
- Proficiency with basic software (i.e. Word, Excel, Access, Outlook, PowerPoint, etc.).
- Willingness to work evenings, weekends, and in various community settings (e.g., clinics, bars, shelters).
- Access to reliable insured transportation required. Valid Ohio driver's license, ability to meet LSS insurance standards.
- Current CPR and first aid certificate or willingness to be trained.
