In this role, Clinical Pharmacy Specialist with Infectious Diseases will provide advanced clinical pharmacy services at Harris Health System. This individual will serve as a resource to the interdisciplinary team, and other pharmacists while delivering innovative practices and service delivery models that ensure cost effective and optimal medication management in the population we serve. Individual will participate in operational and clinical duties of a clinical pharmacist as assigned and required.
Multi-disciplinary rounds including clinical initiatives, direct patient care and educational/training activities will be performed. Individual will also provide education to technicians, pharmacists, and other health care practitioners. They will serve as a preceptor of pharmacy interns, technicians, and ASHP-accredited pharmacy residents.
Minimum Qualifications
Degrees: Must have a Bachelor's Degree in Pharmacy or Pharm. D. degree from an accredited school of pharmacy
License & Certifications:
- Texas Pharmacist License or eligible for Texas Pharmacist License; must obtain licensure within 90 days of employment.
- Texas Pharmacist Preceptor Certification within 90 days of employment
- ACPE IV Sterile Certification
- Board of Pharmacy Specialties Certification required or obtain within 18 months of employment. Must maintain national certification such as Board of Pharmacy Specialties If certification is lapsed, employee has six months to rectify for new certification period.
- American Heart Association Basic Life Support (BLS) certification within 90 days of employment.
- Advanced Cardiac Life Support (ACLS) with course accredited by the American Heart Association (AHA) or per LRC will be required for inpatient positions prior to or within 6 months of employment
Work Experience:
Completion of PGY1 and PGY2 from an ASHP-accredited residency program; OR
PGY1 from an ASHP-accredited residency program with 3 years of progressive clinical practice within specialty area
Communication Skills: Above Average Verbal (Heavy Public Contact), Writing/ Reports
Proficiencies: MS Word, P.C.
