JOB DESCRIPTION: CLINICAL COODINATOR
Responsibilities: The Clinical Coordinator supports field clinicians by providing excellent patient care. Screens incoming telephone calls, emails, and Microsoft Teams messages, and facilitate communication between patients / families and other team members. Responsible for scheduling patient visits for clinical field team, including nurses, home health aides (HHAs), music therapists, chaplains, and medical social workers. Communicates with Clinical Managers and Directors to identify staffing problems and makes recommendations to alter staffing patterns to ensure effective, efficient, and safe patient care.
Displays a high level of empathy, patience, and professionalism at all times. Attends daily stand-up meetings and prepares agendas for IDG meetings. Maintains and updates employee roster, billing roster and patient census records daily and checks for accuracy. Responsible for ensuring office volunteers have projects to complete on a weekly basis and completes office related tasks as needed.
The Clinical Coordinator will assist the clinical team in requesting, clarifying, faxing, and tracking of orders. Input orders, PRN visits, and obtain supporting documentation as necessary. Upload orders into the electronic medical records as appropriate. Facilitates and follows-up on all incoming and outgoing orders, faxes, and other required documentation such as Plans of Care, Attending Physician Authorization forms and Medical Information Request Forms. Orders Durable Medical Equipment and Medical Supplies as required. Orders and monitors medical supplies to ensure adequate stock available. Maintains organization of the medical supply room per company standards and OSHA requirements.
Covers for other Clinical Coordinators such as lunch, team meetings or other short intervals of absence. Maintains communication between field staff, mortuaries, Coroner, and families. The incumbents may be requested to perform job-related tasks other than those stated in this description.
Required Qualifications:
· High school graduate or equivalent.
· Excellent customer service skills.
· 3 years secretarial / administrative assistant experience.
· Excellent communication skills.
· Able to handle multiple tasks.
· Microsoft Excel and Microsoft Word & Computer literate.
· Compassionate, organized, motivated team player.
Preferred Qualification:
· Experience with staffing and scheduling systems.
· Homecare Homebase (EMR) knowledge.
