About The Position
**Job Title:** Registered Nurse - Case Manager **Job Specialty:** Case Management **Job Duration:** 13 weeks **Shift:** Day 5x8-Hour (08:00 - 16:30) **Guaranteed Hours:** Not specified **Experience:** Minimum 2 years of case management experience required; previous charge experience preferred **License:** Must have a current license **Certifications:** BLS required **Must-Have:** - Previous travel assignment experience - Ability to collaborate with hospitals, physicians, etc. - Capability to manage care plans addressing Social Determinants of Health **Job Description:** - Work as a Registered Nurse specializing in case management. - Develop comprehensive care plans for vulnerable populations, including Medicaid recipients. - Conduct comprehensive assessments and manage care within electronic medical records. - Ensure documentation adheres to quality and contract requirements. - Provide care for a diverse age range, from newborns to geriatric patients. - Coordinate care and discharge planning using InterQual Criteria and Milliman Guidelines. - Must be prepared to handle chronic diagnoses, such as high-risk pregnancy, DM, CHF, COPD, asthma, and behavioral health. - Engage in regulatory tasks including coding, billing, and adherence to standards by CMS, DRG, ICD 10, OSHA, The Joint Commission, and others. - Face-to-face interactions required at designated wellness centers. - Office-based work setting with business casual dress code. - All employees are required to have the flu vaccine, with no exemptions. (Note: COVID-19 vaccine not required for this role.)
