The Case Manager will act as the contact person, advocate and resource to clients who are leaving the Bristol County Sheriff’s Office and entering their re-entry program.
$20-22/hour, commensurate upon education/experience.

Duties & Responsibilities
- Meet with the BCSO staff and Justice Involved individuals (JI-Rs) within 90 days of discharge and reentering the community.
- Develop a comprehensive reentry plan in concert with BCSO Reentry team that addresses post release providers and a plan for immediate and ongoing access to chronic or infectious disease needs, MOUD when appropriate, housing and food security.
- Ensure that all services identified in the comprehensive plan are in place and ready for utilization upon release.
- Will coordinate services with SSTAR’s services as appropriate such as the Health Center for physical care, ABH for mental health/SUD care, Community Health for social service care, etc.
- Provides feedback to BCSO staff on the client’s progressive and commitment to their individual plan for services.
- Adheres to the work schedule. Obtains supervisor approval in advance of schedule conflicts as well as late arrivals or early departures.
- Records all hours worked by the last day of the pay period.
- Follows the incident reporting policy and procedure in reporting any unsafe/unusual safety issues.
- Treats patients, and colleagues with dignity and respect; honors each person’s right to privacy/ maintains very strict patient and colleague confidentiality.

Required Experience:
Education & Experience
- GED/HS required. BA in social work or related field, preferred
- 1-2 years’ experience in similar role
- Knowledge of SUD and mental health counseling
- Ability to function independently and work collaboratively
