Their functions include concurrent and retrospective review to assure appropriate utilization, assessment, prevention of denials and coordination of discharge options. They are required to follow specific criteria sets to assure regulatory compliance. They coordinate safe discharge plans between the health care delivery team, patients, families, insurers and continuing care agencies.Education/Experience: Graduate of an accredited school of nursing BSN preferredCertification/License: Current Massachusetts RN licensure Certification in Case Management preferredPatient Age Served: Provides direct patient care to:Neonates (under 1 month)Pediatric (1 month to 12 years)Adolescent (12-17 years)Adult (18-64 years)Geriatric (over 65 years)As a health care organization, we have a responsibility to do everything in our power to care for and protect our patients, our colleagues and our communities.
