Description and Responsibilities:
The Youth and Young Adult Group Residence and the Young Adult Supported Living programs support individuals (males and females) referred by the Department of Children and Families. The group residence serves seven young adults 17-22 years old while the supported living program serves two young adults 18-22 years old. The programs focus on building independent living skills and supporting the transition to young adulthood. Staff focus on goals such as living within a budget to save money, food shop and cook, do their own laundry, utilize transportation, complete an educational program, and obtain employment.
The Case Manager provides support and assistance to the youth served at the program with developing and practicing social skills and social cues, coordinating day supports, leisure and recreational opportunities and financial management.
Other Key Responsibilities:
- Submit written documentation daily, as required, (i.e.: service notes, incident reports, quarterly reviews, etc.)
- Provide support and assistance with medical appointments
- Assist in the development of an individualized treatment plan (ITP).
- Implement interventions outlined in the ITP to assist working toward goal and objective completion.
- Actively participate in the completion and training of life skills and activities of daily living.
- High School Diploma, GED or equivalent, required.
- Minimum of one year experience working with youth and young adults, required.
- Valid Drivers License and acceptable driving record, required.
- Reliability, willingness to learn, and being open to new opportunities.
