Adult Case Manager - Franklin Office
Riverbend Community Mental Health, Inc., one of the largest providers of community-based mental health services in New Hampshire, is seeking a full-time Case Manager to join our talented multi-disciplinary team working with adults living with severe and persistent mental illness. This position is based out of our Franklin, NH office.
As a Riverbend Case Manager, your primary responsibilities will be working closely with clients in our Community Support Program to coordinate treatment — assisting clients in obtaining the services they need and facilitating communication with the treatment team within Riverbend and at outside agencies. This is a great opportunity to work on the front lines of community-based mental health services, and help our clients achieve optimal recovery and independence.
Responsibilities
• Help clients and family understand treatment options and make informed decisions
• Work with treatment team to facilitate case coordination and information sharing
• Participate in the development and implementation of client treatment goals
• Work with clients to build symptom management skills
• Promote effective utilization of clinical resources and other social supports
Requirements
• Bachelor’s degree in Psychology, Social Work, Education, Rehabilitation, or Human Services
• Excellent communication, problem-solving, time management, and organizational skills
• A valid driver’s license, personal vehicle, and auto insurance
