How You Will Make a Difference

The Care Transition Navigator is the face of VitalCaring when interacting with referral sources. In this role, you orchestrate the perfect balance of patient care and operational efficiency, ensuring the patient experiences a smooth transition home. As a result, your clients view VitalCaring as their preferred home health partner that provides compassionate, personalized home health care to their patients.  

As a Care Transition Navigator, you will: 

• Contribute to a culture of accountability and teamwork 
• Deliver exemplary care to patients along the care continuum 
• Cultivate and nurture relationships with referral sources 
• Champion communication among and between care teams 
• Own individual sales goals

Compensation/Earning Potential: salary range is $70,000 - $85,000 with unlimited commission potential

Experience to Deliver on our Mission 

• RN license, valid state driver's license, and reliable transportation. 
• Two years clinical experience; one year of home health or healthcare sales experience preferred 
• Knowledge of business and fiscal management, governmental regulations, and accreditation standards. 

• Established healthcare contacts with data of achieving referral goals within the market preferred.
