POSITION DESCRIPTION:
The Care Manager Preceptor is responsible for precepting and mentoring the Care Manager team and providing their orientation and training. This position identifies training needs, assists in developing training material and delivers on-going training. This includes accountability for efficient patient/workflow, customer satisfaction, staff satisfaction, productivity and achievement of operational goals as defined by practice leadership. Responsibilities may also include providing care management and care coordination for adult patients with complex illness under minimal supervision. Serves in an expanded health care role to collaborate with specialists, members of the health care team and patients/families to ensure the delivery of quality, efficient and cost-effective health care services.
ESSENTIAL JOB FUNCTIONS:
Works with Quality leadership to continuously evaluate processes, identify problems and propose/develop process improvement strategies to enhance care management and Patient Centered Medical Home delivery of care model.Reviews current literature regarding effective engagement and communication strategies, care management strategies and behavior change strategies and incorporates into clinical practice.Works with Care Manager Leads in the assessment of new employee learning needs and development of employee orientation and education plans to ensure consistency in implementation to all new care managers.Coordinates and facilitates onboarding needs for all new care managers, evaluates orientee's progress and provides ongoing constructive feedback to both orientee and Care Manager Lead in a timely manner.Assists in the coordination, planning, evaluation and integration of clinical education and professional development activities to foster and maintain care management best practices.Integrates patient/program educational activities into care manager meetings and continuing education programs.Completes care manager audits regularly to support Care Manager Lead evaluation and identifies care managers that would benefit from additional mentoring. Maintains required documentation.Collaborates with department leadership in developing policies, standards, procedures or protocols, as appropriate.Facilitates ongoing development of organization, time management and overall care management skills of the care managers.Evaluates and manages day-to-day workflow and adjusts to ensure increased efficiency.May provide care management and care coordination for adult patients with complex illness under minimal supervision.Serves in an expanded health care role to collaborate with specialists, members of the health care team and patients/families to ensure the delivery of quality, efficient and cost-effective health care services.Supports other offices, attends required meetings and training, and participates in committees as requested.Performs other duties as assigned and works within scope of certification/licensure.

ESSENTIAL QUALIFICATIONS:
EDUCATION: Bachelor of Science in Nursing. In addition, prefer completion of self-management support training.
CREDENTIALS/LICENSURE: Valid, unrestricted RN license in the State of Michigan; CCM Certification is required within 6 months of taking on this role, if not already acquired. Valid CPR certification.
MINIMUM EXPERIENCE: 3-5 years of experience within the past 5 years that includes adult medicine in primary care/ambulatory care, home health agency, skilled nursing facility or hospital medical-surgical setting, and NextGen Care Management Template experience, preferably with case management and continuous quality improvement experience. Also preferred: experience as participant in continuous quality improvement.

MINIMUM PHYSICAL EXPECTATIONS:
Physical activity that often requires keyboarding, phone work and charting.Physical activity that often requires extensive time working on a computer.Physical activity that often requires handling and lifting patients walking, standing, bending, stooping, reaching, climbing, kneeling and/or twisting. Physical activity that may require lifting, pushing and/or pulling up to 20 pounds.Specific vision abilities required include close vision, depth perception, peripheral vision and the ability to adjust and focus.Manual dexterity sufficient to operate a keyboard, photocopier, telephone, calculator and other office equipment.Must hear and speak well enough to conduct business over the telephone or face to face for long periods of time in English.
MINIMUM ENVIRONMENTAL EXPECTATIONS:
This job operates in a medical facility and requires regular walking to various locations around the hospital/clinic. Employees will be working where there is patient care equipment. Hazardous materials, including bloodborne pathogens and bodily fluids are also present. Exposure to sharps, x-rays, patients' conditions and some unpleasant sights, smells and contagious diseases is possible. This position requires significant interaction with people (many of whom are scared, hurt and/or ill) which can be stressful and result in competing priorities.
