Bilingual Residential Program Coordinator
Job Title: Bilingual Residential Program Coordinator- Family Caregiver, Companion Homes, Host Homes
Department: Residential
Reports to: Program Manager
Pay: $22/hour
FLSA: Non-Exempt
Schedule: Full-time, Monday-Friday. This is a hybrid position.
Purpose of Position- The Bilingual Program Coordinator supports individuals with developmental disabilities as part of a team that may include nursing services, behavioral and mental health services, adult community services, employment services, and representative payee services. The Coordinator will support the program manager in managing the ongoing operations of residential settings, including Family Caregivers, Host Homes, and Companion Homes. The Bilingual Residential Program Coordinator works with Spanish-speaking individuals and families to access available resources and prevent barriers to care. Ensures program quality, maintains regulatory compliance, and contributes to the financial viability of the residential settings. Provides monitoring and support to the setting to safeguard the health, safety and wellbeing of individuals living in the residential setting to ensure a high quality of life. Maintains professional relationships with all individuals receiving services, contractors, and stakeholders.
Essential Duties/Responsibilities
- Supports the Program Manager in ensuring program quality of residential settings.
- Supervises Providers (Family Caregivers and Host/Companion Home Providers), including training, care planning, and ongoing support.
- Manages a caseload of individuals with intellectual and developmental disabilities who live in the community with family or contracted providers.
- Ensures implementation of each individual’s Person-Centered Support Plan, attends care team meetings, etc.
- Oversees the individual's care in the residential setting.
- Completes in-person monitoring visits to assure the safety of the environment and confirm regulatory compliance.
- Promotes independence and person-centered care. Ensures that all individuals are treated with respect and dignity.
- Represents the department in communicating with providers, serving as a community liaison, etc.
- Reviews all behavioral incidents, accidents, and medical emergencies, referring to the Program Manager when necessary.
- Supports Program Manager in effectively resolving concerns and complaints.
- Aids the Program Manager in developing and implementing site-specific efficiencies.
- Assists the Program Manager in maintaining regulatory compliance of Host Home/Companion Home operations.
- Completes regular in-person monitoring visits for every individual on their caseload.
- Maintains current, up-to-date knowledge of all regulatory issues and changes.
- Ensures program compliance with all standards and regulations, including Imagine! policies and procedures.
- Monitors and tracks data to ensure programmatic compliance.
- Regularly seeks to develop program-wide Quality Improvement initiatives.
- Fulfilling on-call responsibilities, including conflict resolution and emergency situations that meets Imagine! standards.
Other Duties/Responsibilities
- Ensures the health and safety of all clients at the homes.
- Ensures presentation, including cleanliness and organization, of home(s).
- Maintains professional relationships with Residential stakeholders.
- Responsible for customer satisfaction for assigned home(s).
Job Qualifications:
- Knowledge, Skill, and Ability:
- Fluency in English and Spanish
- General knowledge of the development disabilities system in Colorado
- Demonstrated skill in working with adults with intellectual and developmental disabilities (or similar population) in a residential setting.
- Ability to work independently and prioritize tasks and goals
- Effective written and verbal communication skills with individuals and groups at all professional levels.
- Effective and creative negotiator and problem solver.
- Effective training skills.
- Ability to work effectively with various technology including word processing software, internet based databases, assistive technology, etc.
- Possession of a valid driver’s license and ability to meet Imagine! driving requirements.
- This position requires regular use of a personal mobile device such as a smartphone or tablet.
- Training/Education:
- Bachelor’s Degree in the Human Services field highly preferred. May be substituted with equivalent combination of education and experience
- Satisfactory completion of job-specific and/or site-specific training.
- Experience
- One – Three years of experience working in the field of intellectual and developmental disabilities or similar human services field required.
- Supervisory experience preferred
- Working Environment/Physical Activities
- Ability to lift 50 pounds.
- Effective with shifting roles, responsibilities, and expectations in a changing environment.
- Ability to respond to emergency situations as-needed.
