Responsibilities:The Behavioral Health Social Work Lead is a key member of the interdisciplinary team who provides and oversees the provision of behavioral health assessments, crisis intervention, and SBIRT services to patients in the Emergency room and elsewhere as needed. Responsibilities include bio-psychosocial assessments and providing intervention and treatment as indicated. Identifies high-risk factors (homelessness, mental health, substance abuse) of patients that impact health status and discharge planning. Partners with the patient, family, healthcare team, and community agencies in the safe transition of care to the next most appropriate level.
- Supervises the Behavioral Health and SBIRT programs and the work of the Peer Recovery Specialists and Behavioral Health Social Work Staff.
- Provides Behavioral Health assessment and crisis intervention to mental health patients in the Emergency Department, and elsewhere as needed.
- Collaborates with the ED attending provider to develop recommendations for the next level of care for mental health patients.
- Recommends and documents psychiatric plan for disposition for mental health patients in the Emergency Department.
- Develops and implements effective discharge plans for all patients on referral, initiating service within departmental policies, involving patient and family in formation of plan and achieving the discharge within the appropriate length of stay.
- Participates effectively in interdisciplinary plan of care, including communication with physicians and staff, and participation in multidisciplinary rounds.
- Provides counseling for patients and families when appropriate as part of plan of care, effectively using crisis intervention therapy model.
- Provides bio-psychosocial evaluation of Emergency Department patients referred for substance abuse and/or mental health treatment at discharge, and makes appropriate arrangements for discharge disposition.
- Provides accurate community resource linkage as part of discharge plan, identifying correct resources, educating patient on use of resources and completing all requirements of effect successful referral.
- Documents social work interventions accurately, consistent with departmental guidelines for content and timeliness.
- Must possess a degree in Social Work
- MSW with 2 years post graduate experience in health care setting, supervisory experience preferred
- Must be licensed by the Maryland Board of Social Work Examiners at the LCSW-C Level or as a LCPC
