• Must have active state license
• BC required
• Weekdays, Days, Call, Weekend call
• BLS needed
• Average of 3 - 6 cases per day
• General, ortho, ortho spine, GYN, urology, GS, endoscopy, some peds cases if the physician prefer
• Credentialing needed
• DEA needed
