BROOKSIDE MULTICARE NURSING CENTER
DEPARTMENT: Adult Day Health Care Program
JOB TITLE: Registered Nurse Coordinator
REPORTS TO: Director
SUPERVISES: Registered Nurse, Recreation Therapist, Program Assistants, Social
Worker
COORDINATES WITH: Food Service, Environmental Services, Plant Operations
EDUCATION/EXPERIENCE/QUALIFICATIONS:
1. Current New York State Licensure, B.S. preferred
2. Minimum of 5 years work experience including practice, teaching and supervision
3. Knowledge of gerontology, psychiatric and restorative nursing
4. Demonstrates leadership abilities
5. Familiarity with Long Term Care policies
PHYSICAL/MENTAL QUALIFICATIONS: No hearing or sight impairment that would render the person unable to function in their position.
SCOPE: Responsible for promoting the delivery and coordination of comprehensive care to participants, through the activity of assessment planning and providing direct nursing care in accordance with the policies and standards of Adult Day Care.
DUTIES AND RESPONSIBILITIES:
1. Assists in the development, supervision and implementation of the registrant’s care plan.
2. Provides direct supervision for registrant care, and actively engages in care when necessary.
3. Keeps private physician and Director informed of registrant’s status.
4. Reviews regiment of registrant at regular intervals with Program Director.
5. Assists in the nutritional aspects of the program.
6. Assists in all program activities as needed.
7. Provide health education for staff, registrants and families.
8. Identifies health needs and provides for appropriate care.
9. Provides a nursing evaluation of each registrant within 30 days of admission.
