Bay Aging is seeking an Options Counselor for ourVeteran Directed Care Program in St. Croix, Virgin Islands. The OptionsCounselor provides person-centered counseling services to Veterans enrolled inthe Veterans Directed Care(VDC) program. VDC is a program that serves eligibleveterans of any age who are at risk of nursing home placement. The OptionsCounselor is responsible for working with the Veteran to identify their needsand develop a spending plan to meet those needs. He or she works with the Veteranto assess, identify, coordinate and monitor supports and services on an ongoingbasis. The Options Counselor monitors all VDC spending along with the Veteran,the Financial Management Service (FMS) provider and the VAMC. They reportdirectly to the Director, Community Living Programs.
Salary: $48,500/yr-$59,000/yr.
Bay Aging's Veteran Directed Care Program provides support to veterans in multiple states throughout the United States. It is headquartered in Urbanna, Virginia. We are excited to be expanding to the Virgin Islands. The VDC program is an award winning program supporting veterans throughout the nation. Awards received: Excellence in Financial Management Services (2019); Healthcare Achievement (2019), Aging Achievement (2018); VA Approved Financial Management Services Provider since 2011 and VA Approved Directed Care Provider since 2008.
ESSENTIAL FUNCTIONS:
1. Home visits (50% including travel time): This includesinitial assessments in the client’s home, development of a spending plan, assisting the Veteran with the processof becoming an employer, facilitating new hire appointments, conductingmonthly, quarterly, semi-annual, and annual required contacts and reassessmentsas needed. Provide training and remediation as necessary related to employer responsibilities.Suggested maximum case load 40 veterans perOptions Counselor.
2. Office work (50%): Follow-up on referrals and service implementations forclients. Maintains, updates, and monitors clients' confidential information,services and fildatabasen data base program as required. Performs fiscal dutiesas required under Veterans Directed Care program. Maintains accurate records ofexpenditures, schedules amp; generates monthly reports as required;responsible for satellite office administration as assigned.

· Performs a range of tasks and duties independently,without direct supervision. Relies on experience and judgment to plan andaccomplish goals, advocate for clients, families, community partners.
· Maintainstravel schedule and related expense reports. Organizes and prioritizes scheduleand duties independently.
· Provides regular reports and updates to supervisor inconflict resolution if warranted.
KNOWLEDGE, SKILLS, AND ABILITIES
· MicrosoftOffice programs (e.g., Word, Excel, Outlook, PowerPoint) and standard officeequipment
· Dataentry and evaluation

· Decisionmaking:Work independently whennecessary. Handle a variety of tasks autonomously with regular reporting tosupervisor and co-workers.
· Detail oriented — maintain schedule, services, communications amp;advocacies for clients.
· Establish and maintain trusting relationships withclients and their families in all circumstances Interact and work well withother agency departments, staff amp; employees.
WORKING ENVIRONMENT:
Office-based position with minimal physical requirements (ability to lift15 lbs., walking and climbing stairs). Requires regular independent travel.Approximately 90% of the essential functions of the position can be performedremotely.
MINIMUM QUALIFICATIONS:
3 or more years of experience working with seniors, disabled adults,human services or general case management field.
Bachelor’s degree in social work, human services or medical fieldpreferred.
CriminalHistory background check required.Must possessa valid driver's driver's license and proof of current PPD test amp; Fluvaccinations.
