0.6 FTE working 24 hours per week. Hours: Rotating biweekly Float schedule
We have a night/weekend shift differential of $2.65/hr. This starts at 7pm until 7a on week days M-Th and for the weekends, it is an additional $2.65/hr for the whole weekend starting at 7pm Friday night through Monday at 7AM.
Provides mobile person-centered behavioral health care services to clients across the life span who are experiencing a behavioral health crisis. Completes suicide and violence risk assessments. Collaborates with internal and external providers to identify and address immediate safety needs. Facilitates voluntary and involuntary inpatient psychiatric stays when indicated. Provides risk management safety planning and linkage to ongoing services. Provides ongoing supportive services to client who are at risk of re-entering a behavioral health crisis. Supports and promotes the Mission, Vision and Guiding Principles of the Human Services Department. Performs in a manner consistent with the HSD Professional Standards and “Commitment to My Co-Workers.”
60%- Provides mobile person-centered behavioral health care services for clients experiencing a behavioral health crisis
1. Assess clients in crisis to determine least restrictive disposition that will assure client safety
2. Assess for voluntary admission options for individuals without a funding source.
3. Authorize chapter 51.15 emergency detentions when appropriate.
4. Coordinate admissions, when clinically indicated, to the appropriate receiving facility.
5. Coordinate medical clearance if required with police, referring facility, and hospital ER staff.
6. Work with individuals and natural supports to create person centered safety plans when clinically indicated and in consultation with the crisis supervisor.
7. Create person-centered crisis response plans, crisis plans
8. Provide individual, family, and/or group brief psychotherapy
9. Coordinate client care and linkage to ongoing behavioral health services as assigned.
20%-Consult and assist with recommendations to law enforcement agencies about those
individuals who are experiencing a Substance Use Disorder related crisis.
1. Assess for and facilitate detox facility admissions under 51.45
2. Provide post-discharge risk assessments for clients discharging from a detox facility.
3. Coordinate referrals to Rock County Connections staff for ongoing linkage to services.
10%- Conduct correctional detox, suicidal, and homicidal assessments.
1. Provide behavioral health risk assessment at the Rock County Jail and Youth Services Center
2. Coordinate the admission process with facility staff when psychiatric hospitalization is clinically indicated.
5%- Ensure program and medical record compliance
1. Follow all departmental policies and procedures
2. Document client care in accordance with policies, procedures, and regulatory guidelines
3%- Participates in all clinical and administrative meetings or activities as required
2%- Will perform other duties as assigned
